"Doctor, is the cap really necessary?" It is one of the most frequent questions after a root canal, usually asked once the pain has gone. The short answer: for back teeth, yes, almost always. For front teeth, sometimes a filling is enough. Here is why.
What a root canal does to the tooth
Root canal treatment removes the pulp — the nerve and blood supply — from inside the tooth. The tooth survives, but it no longer receives moisture from inside and becomes more brittle. Access to the canals also removes some of the tooth's core, and most root-treated teeth already had large cavities or fillings.
The result is a tooth that works but is weaker, especially under the heavy forces of chewing on molars and premolars.
What the cap (crown) does
A crown covers the whole tooth above the gum, holding it together like a protective shell. It:
- Prevents the tooth from splitting under chewing forces
- Seals the tooth against bacteria re-entering the canals
- Restores full chewing shape and contact with neighbouring teeth
- Returns the tooth to a natural colour and shape
A root-treated molar without a crown has a high chance of fracturing — sometimes below the gum, where the only option left is extraction.
When a crown can be skipped
Front teeth (incisors) with small access openings and plenty of healthy tooth remaining can sometimes be restored with a strong tooth-coloured filling. Your dentist will tell you honestly whether the remaining structure is enough. If the tooth is discoloured after treatment, a crown or veneer may still be chosen for appearance.
Which type of cap
- Zirconia — strong and tooth-coloured, suited to back teeth and most front teeth.
- Porcelain-fused-to-metal (PFM) — a proven option with a metal core.
- All-ceramic / lithium disilicate — the most natural look for front teeth.

Timing
The crown should be fitted soon after the root canal — ideally within a few weeks. Every month of chewing on an unprotected tooth adds risk.
