Losing most or all of the teeth in a jaw used to mean a removable denture. All-on-4 changed that: a complete arch of fixed teeth supported by just four implants, with provisional teeth fitted within days.
How All-on-4 works
Four implants are placed in one jaw. The two front implants go in vertically; the two back implants are tilted at an angle so they anchor in the denser bone at the front of the jaw and avoid the sinus (upper) or the nerve (lower). A fixed bridge is then screwed onto the implants.
The angled placement is what makes the concept work: it uses the bone most people still have, so bone grafting is often unnecessary, and four implants provide enough support for a full arch.
The timeline
- Consultation and CBCT scan. The 3D scan shows bone volume and lets the implantologist plan positions digitally.
- Surgery day. Any remaining failing teeth are removed and the four implants placed under local anaesthesia.
- Provisional fixed teeth within 3 to 5 days. A fixed provisional bridge is attached so you leave with teeth, not a denture.
- Healing, 3 to 4 months. The implants integrate with the bone.
- Final prosthesis. A stronger, definitive bridge replaces the provisional one.

All-on-4 vs All-on-6
All-on-6 uses six implants per jaw. It is chosen when bone quality and jaw shape allow and extra support is wanted. The decision comes from the scan, not from preference. We compare them in All-on-4 vs All-on-6.

General health, smoking and bone volume are assessed first. Diabetes that is well controlled is not a barrier.

Common questions
Is All-on-4 painful? Surgery is done under local anaesthesia. Swelling and soreness for a few days are managed with medication and a soft diet.
How long do All-on-4 implants last? With good hygiene and check-ups the implants are intended to last for decades; the bridge may need refurbishment over time.
Can I eat normally with the provisional teeth? A soft diet is advised during the healing months to protect the integrating implants; normal chewing resumes with the final bridge.
