Patients researching implants come across two terms — basal and conventional — and the marketing around speed can make the choice confusing. The honest summary: they anchor in different bone and follow different protocols, and the right one depends on your case.
Conventional implants
A titanium post is placed into the jawbone where the tooth root was, and the bone is allowed to bond to it before the final crown is fitted. A single-tooth implant takes around 10 to 14 weeks in total.
For a full arch, All-on-4 and All-on-6 use four or six implants, with provisional fixed teeth fitted within 3 to 5 days and the final prosthesis about 3 to 4 months later.
This is the approach with the longest documented record — global studies show success rates above 95 percent at ten years — and it is what most implant systems, components and research are built around.
Basal implants
Basal implants anchor into the denser basal bone rather than the softer bone where the tooth root sat, and are often promoted for immediate loading, sometimes within 72 hours. They are used in situations where bone volume is limited, and proponents argue they can avoid grafting.
They remain more specialised, and the long-term evidence base is smaller than for conventional implants.
What actually decides the choice
- Bone volume and quality, shown on a 3D CBCT scan.
- How many teeth are being replaced.
- Your general health and healing capacity.
- What the long-term plan is, including what happens if a component ever needs replacing.
Speed alone is a poor reason to choose an implant that will have to last decades.
Questions to ask any clinic
- Which system do you propose, and why for my case?
- What is the documented track record of that system?
- Will components still be available years from now?
- What happens if the implant fails?
Common questions
Is faster better? Not by itself. The bond between implant and bone is what carries the load long term.
Can I avoid bone grafting? Sometimes, depending on the anatomy — a scan gives the answer.
