Step 1
Click the link Dental Assessment Form in the clinic web site. Once the form opens fill in your personnel details. Those fields marked mandatory must be filled. Please give a brief description of your dental problems and requirements. You may look into the dental chart which helps you to clearly specify each tooth you wanted to mention by a number
If you do not feel comfortable to understand the dental chart and details, try to explain your problems in words.
Step 2
Within 48 hours Dr Jay sinojiya will personally reply your enquiry via email. All the lead questions will be forwarded for you to answer. Most certainly Dr Jay Sinojiya will request for an OPG X-ray for him to derive a TREATMENT PLAN AND COSTING
OBTAINING AN OPG X RAY
Call the nearest dental clinic or your dental surgeon and find out how you can get an OPG X-ray. Once you receive the X-ray you can either post it in the below address.

If you don't want to post it then scan the x-ray covert that into a jpg format and send it as email as small file size. If you cannot manage the OPG x-ray in your country, we can get it done here when u arrive, and we can start the treatment accordingly.
Step 3
On receipt of the OPG X-ray the clinic will formulate a treatment plan and treatment cost for your consideration... At this point we strongly recommend you to call Dr JAY SINOJIYA by telephone on a mutually convenient time to discuss your treatment details AND DATE AT WHICH YOU CAN ARRIVE FORTREATMENT
Step 4
After mutual consultation with the clinic and you, your schedule is sketched and appointments are made with Dr JAY SINOJIYA and team.
